摘要: 目的 全科医生队伍建设是深化医改、构建完善分级诊疗体系的重要内容,本文总结分析安徽省全科医学教育培训工作的实践,对如何促进全科医生队伍建设提出经验参考和政策建议。 方法 回顾分析安徽省全科医学教育培训主要政策、基本情况、存在问题等,提出工作建议。 结果 全科医学教育培训存在的主要问题有:培训成效难以显现,质量有待提高;全科医生执业制度推进滞后、取得培训合格证书人员注册率较低、流失率偏高;规范化培训在整体培训中比例、报名人数和录取分数呈现"三低";政策宣传不到位,激励措施缺乏,参培积极性不高等。全科医生队伍建设存在的主要问题有:全科医生队伍总体上数量匮乏、质量不高,全科医生既未得到社会普遍认同,也未得到业内普遍认可(不被重视);对全科医学认识不到位,在全科专业科室设置和人员配备上重视程度不高;全科医生使用激励保障机制不完善,全科医生岗位吸引力差;家庭医生签约服务、分级诊疗制度推进难度大,效果不理想等。 结论 建议从五个方面加强全科医学教育培训和全科医生队伍建设:加强组织领导;推进执业注册;强化规范培训;重视学科建设;加强宣传引导。Abstract: Objective Team construction of general practitioner is one of the important content of deepening medical reform and improving hierarchical medical system. The aim of the paper is to summarize and analyze the practice of general practice education and training in Anhui province, and put forward experience references and policy suggestions for the team construction of general practitioner. Methods We reviewed the main policy, basic situation and problems for the education and training of general practice in Anhui province. Results The main problems in general practice education and training included:not obvious effect; lower training quality; delayed practice system of general practitioners; low registration rate and high turnover rate in qualified certificate personnel; low proportion of standardized training, number of applicants and admission score; insufficient policy-propaganda; lack of incentives, poor participation initiative, and so on. The main problems in general practitioner team construction included:lack of quantity and lower quality of general practitioners; lower level of social support and generally accepted by colleagues in their industry; disregard on the establishment and staffing of department because of the lack of understanding about the general practice; not perfect excitation and protection mechanism for general practitioners and unattractive jobs; difficult implementation and non-ideal result for family doctor contract service and hierarchical medical system. Conclusion The following measures should be performed to strengthen the training and team construction of general practitioner:strengthening the organization and leadership;promoting practice registration; strengthening standard training; paying more attentions to discipline construction;strengthening publicity and guidance.
