Comparative analysis of the effect of minimally invasive internal fixation and open reduction internal fixation on thoracolumbar spine fracture

摘要: 目的 探讨微创内固定术与开放式固定术对胸腰段脊柱骨折疗效对比。 方法 选取2016年5月-2017年5月安吉县第三人民医院收治的胸腰段脊柱骨折患者150例,根据治疗方法不同分为2组。对照组应用开放式固定术治疗,研究组应用微创内固定术治疗,对比2组患者手术结果、影像学指标以及VAS评分。 结果 2组患者的手术时间对比差异无统计学意义(P>0.05),研究组患者的切口长度、手术出血量、术后引流量以及住院时间均显著低于对照组(均P<0.05);治疗前2组患者的Cobb's角、椎体前缘高度以及椎体矢状面指数差异无统计学意义(P>0.05),治疗后研究组患者的Cobb's角显著低于对照组,同时椎体前缘高度以及椎体矢状面指数显著高于对照组(均P<0.05);治疗前2组患者的VAS (视觉模拟法)评分差异无统计学意义(P>0.05),治疗后各个时间点研究组患者的评分均显著低于对照组(P<0.05)。 结论 胸腰椎段脊柱骨折通常是高能量外力诱发的脊柱骨折,在伤情严重的情况下会影响到马尾神经以及脊髓,而胸腰段脊柱骨折患者应用创内固定术治疗有利于提高临床治疗效果,改善患者的影像学指标,降低患者疼痛情况,临床上应当推广应用。Abstract: Objective To compare the effect of minimally invasive internal fixation and open reduction internal fixation on thoracolumbar spine fracture. Methods Total 150 cases of thoracolumbar spine fracture from May, 2016 to May, 2017 year in May in our hospital were selected and randomly divided into two groups, the control group received open reduction internal fixation treatment, while the study group received minimally invasive internal fixation, The surgical results, imaging indexes and VAS score were compared between the two groups. Results There was no significant difference in operation time between the two groups (P>0.05). The incision length, bleeding volume, postoperative drainage volume and hospitalization time in the study group were significantly lower than those in the control group (P<0.05);before the treatment, there was no significant difference in Cobb 's angle, anterior height of vertebral body and vertebral sagittal index between the two groups (P>0.05); after the treatment, Cobb's angle in the study group was significantly lower than that in the control group, however, the anterior height of vertebral body and vertebral sagittal index were significantly higher than that in the control group (P<0.05); there was no significant difference in visual analogue score (VAS) before the treatment between two group (P>0.05), however, VAS after the treatment at different time points in the study group was significantly lower than the control group (P<0.05). Conclusion Thoracolumbar spine fractures are usually induced by high energy force, and cauda equina and spinal cord can be affected in case of serious injury, the internal fixation can improve the clinical treatment effect and imaging index, reduce the pain of patients, worth to be popularized in clinic.
