摘要: 目的 探讨高强度线减压自体半腱肌重建喙锁韧带治疗老年患者肩锁关节脱位的方法,观察其临床疗效。 方法 通过回顾研究,收集厦门大学附属福州市第二医院2014年6月-2016年1月,25例Rockwood Ⅲ~Ⅵ肩锁关节脱位的老年患者,年龄在62~71岁,平均年龄64.3岁。所有患者通过小切口暴露锁骨及喙突,建立骨隧道,将双股高强度线及自体半腱肌腱绕喙突下方后引入隧道,重建喙锁韧带,采用VAS评分、UCLA评分、ASES肩关节功能评分,记录关节肩关节活动度,对比术前、术后数据评估其临床效果,同时摄片对比术前后肩锁关节复位情况。 结果 平均随访时间为1.5年,VAS评分从术前4.3±1.2下降到0.8±0.3,明显缓解(t=5.632,P<0.01),ASES评分术前为64.1±7.4,术后为92.3±3.2,有明显改善(t=8.537,P<0.01);UCLA评分由术前22.1±3.9,改善为34.8±2.4(t=4.915,P<0.01);关节活动度均有明显改善。末次随访拍摄肩关节X线片未发现肩锁关节复位丢失。 结论 高强度线减压下自体半腱肌重建喙锁韧带治疗老年性肩锁关节脱位,损伤小,临床疗效满意,术后可获得良好的肩关节功能。Abstract: Objective To determine the functional outcome of high strength wires hang together with autologous tendon to reconstruct coracoclavicular ligment for elderly patients with acromioclavicular joint dislocations. Methods This study is retrospective study, examined elderly patients with acromioclavicular joint dislocations from June, 2014 to January, 2016. Twenty-five elderly patients of Rockwood Ⅲ-Ⅳ acromiocluv, cular joint dislocation were observed, the mean patient age at the time of surgery was 64.3 years (range, 62 to 71 years). All patients were treated with two high strength wires and autologous tendon to reconstruct coracoclavicular ligment. The shoulder joint range of motion before treatment and post treatment were recorded, we compared VAS scores, UCLA scores, ASES scores to determine the function of shoulder joint. Results All were available for follow-up at a mean of 1.5 years. Overall, functional outcome was improved from baseline compared with final follow-up for VAS scores[(4.3±1.2) points vs. (0.8±0.3) points, P<0.01], ASES scores[(64.1±7.4) points vs. (92.3±3.2) points, P<0.01], and UCLA scores[(22.1±3.9) points vs. (34.8±2.4) points, P<0.01]. The shoulder joint range of motion was improved. Conclusion High strength wires hang together with autologous tendon to reconstruct coracoclavicular ligment for elderly patients with acromioclavicular joint dislocations is a good method which has less damage, it can obtain a favorable clinical effect, and have good functional rehabilitation.
