Study on guiding effect of mental health of elderly patients with type 2 diabetes in community

摘要: 目的 人口老龄化加速和2型糖尿病患病率持续上升,以及老年2型糖尿病患者容易出现不良情绪,影响血糖控制和生存质量,探讨对社区老年2型糖尿病患者及家属进行心理健康指导的效果。 方法 2015年底长桥街道社区卫生服务中心纳入慢病管理的糖尿病患者,符合入组标准的采用随机数字表法抽取600例,再按随机序号排列,根据单双数分为干预组和对照组各300例。2组患者均给予糖尿病内科药物治疗、饮食控制、运动疗法、血糖监测、健康教育。干预组在此基础上对患者及家属实施1年的心理健康指导培训,由家属配合医师对糖尿病患者进行综合性心理教育式家庭干预。量表评定时采用双盲法,对2组评定结果进行对比分析。全部资料采用SPSS 20.0统计软件进行数据统计分析,运用描述性统计分析、t检验。 结果 1年后,干预组完成287例(95.7%),脱落13例(4.3%),对照组完成288例(96.0%),脱落12例(4.0%),脱落原因为死亡、搬迁、出国。干预组抑郁自评量表(SDS)、Zung焦虑自评量表(SAS)、空腹血糖(FBG)、餐后2小时血糖(PBG)、糖化血红蛋白(HbA1C)均低于对照组,生存质量问卷(SF-36)量表评分高于对照组(均P<0.05)。 结论 开展社区老年2型糖尿病患者心理健康指导对患者的血糖控制及提高生存质量具有积极影响。Abstract: Objective Accelerating aging of population, the increasing prevalence of type 2 diabetes, and elderly patients with type 2 diabetes prone to adverse emotions, affect glycemic control and quality of life, and to explore the effects of mental health guidance on elderly patients with type 2 diabetes and their families in the community. Methods At the end of 2015, Changqiao Community Health Service Center included diabetes patients with chronic disease management, with 600 cases who met the criteria and were selected by random number table method, and then ranked by random number. According to single and double numbers, the intervention group and the control group were divided into 300 cases in each group. Both groups of patients were given diabetes medical treatment, diet control, exercise therapy, blood glucose monitoring, and health education. On the basis of this, the intervention group conducted a one-year mental health guidance training for patients and their families, and the family members cooperated with the physicians to conduct comprehensive psychological education-type family intervention for diabetic patients. The double-blind method was used to measure the scales, and the results of the two groups were compared and analyzed. All data were analyzed by SPSS 20.0 software for statistical analysis, using descriptive statistical analysis and t test. Results One year later, 287 cases (95.7%) of the intervention group completed the guide and 13 cases (4.3%) were detached, while in the control group, 288 cases (96.0%) completed, and 12 cases (4.0%) were detached. The causes of detachment were death, relocation, and going abroad. Zung Self-Rating Depression Scale (SDS), Zung self-rating anxiety scale (SAS), fasting blood glucose (FBG), postprandial 2-hour blood glucose (PBG), glycosylated hemoglobin (HbA1 C) determination in intervention group were all lower than those in the control group, while the quality of life questionnaire (SF-36) scale in the intervention group was higher than that in the control group (all P<0.05). Conclusion Conducting mental health guidance for elderly patients with type 2 diabetes in the community has a positive impact on patients' blood glucose control and quality of life.
