Serum neurofilament light chain level and its clinical correlation in patients with optic neuromyelitis spectrum disease

摘要: 目的 探讨视神经脊髓炎谱系病(neuromyelitis optica spectrum disorder,NMOSD)患者血清神经丝蛋白轻链(serum neurofilament light chain,sNFL)水平,并分析其与病情轻重的相关性。 方法 收集2015年7月—2018年12月就诊于蚌埠医学院第一附属医院的NMOSD急性期患者42例作为NMOSD组,同期健康体检者42例作为正常对照组,采用扩展残疾状态量表(EDSS)评估NMOSD组疾病严重程度,以电化学发光免疫分析法(electrochemiluminescence immunoassay,ECLIA)检测sNFL水平,比较NMOSD组与对照组间的sNFL水平,及不同亚型的NMOSD患者sNFL水平,采用简单线性回归方法分析患者sNFL水平与EDSS评分的相关性。 结果 NMOSD组sNFL水平[(81.69±35.49)pg/mL]较正常对照组[(16.00±5.61)pg/mL]明显升高,差异具有统计学意义(t’=11.848,P<0.001);AQP4-Ab(+)组sNFL水平[(109.86±31.45)pg/mL]较AQP4-Ab(-)组[(67.61±28.62)pg/mL]明显升高,差异具有统计学意义(t=4.365,P<0.001)。NMOSD组sNFL水平与EDSS分值呈正相关(t=9.108,P<0.001)。 结论 NMOSD患者急性期sNFL水平增高,并与疾病严重程度呈正相关。Abstract: Objective To investigate the serum neurofilament light chain (sNFL) levels in patients with neuromyelitis optica spectrum disorder (NMOSD), and to analyze the correlation between the sNFL levels and severity of disease. Methods Forty-two patients with acute phase of NMOSD hospitalized in the first affiliated hospital of Bengbu medical college were enrolled as the NMOSD group from July 2015 to December 2018, and 42 healthy subjects as the normal control group during the same period. Extended disability status scale (EDSS) was used to evaluate the severity of disease with the NMOSD group, and the levels of sNFL were detected with electrochemiluminescence immunoassay (ECLIA). The sNFL levels of NMOSD group and control group, and the sNFL levels of different subtypes of NMOSD were compared. Simple linear regression was used to analyze the correlation between the sNFL levels and the EDSS scores. Results The sNFL level [(81.69±35.49) pg/mL] in the NMOSD group was significantly higher than that in the normal control group [(16.00±5.61) pg/mL], and the difference was statistically significant (t'=11.848, P<0.001). The sNFL level [109.86±31.45)pg/mL] in the AQP4-Ab (+) group was significantly higher than that in the AQP4-Ab(-) group [(67.61±28.62)pg/mL], and the difference was statistically significant (t=4.365, P<0.001). The sNFL level of NMOSD group was positively correlated with the EDSS score (t=9.108, P<0.001). Conclusion The level of sNFL increased in the acute phase of NMOSD is positively correlated with the severity of the disease.
