Analgesic effect of dexmedetomidine combined with sufentanil on pains caused by burn

摘要: 目的 探讨右美托咪定复合舒芬太尼对烧伤疼痛患者的疗效及对单核细胞P2X7受体表达的影响。 方法 选择2015年12月—2017年12月间浙江省台州医院烧伤科收治的患者152例,按照患者入院顺序编号奇偶数法均分为2组,对照组(76例)行单纯舒芬太尼镇痛治疗,观察组(76例)行右美托咪定联合舒芬太尼镇痛,对比2组患者治疗前后动脉血压等基本生命体征、疼痛评分及外周血单个核细胞P2X7受体表达情况。 结果 2组患者干预前与干预后SBP、DBP及HR均显著下降(P<0.05),但2组组间只有SBP、DBP具有显著性差异(P<0.05),HR和SpO2差异无统计学意义(P>0.05) 。各时段疼痛评分较治疗前均显著下降(均P<0.05),且观察组显著低于对照组(均P<0.05),组内各时间段疼痛评分呈下降趋势,差异有统计学意义(均P<0.05);2组干预后20 min及60 min镇静评分差异有统计学意义(均P<0.05),但干预后5 min 2组患者镇静评分差异无统计学意义(P>0.05);观察组患者治疗前后血清应激反应指标显著下降(均P<0.05),而对照组干预前后差异无统计学意义(均P>0.05);流式细胞学检测显示:观察组患者干预后CD14细胞P2X7受体表达显著降低(P<0.05),而对照组患者P2X7受体表达无明显改善(P>0.05)。 结论 右美托咪定联合舒芬太尼干预烧伤患者镇痛效果明显,不加重心功能抑制,且显著减轻应激反应,降低P2X7受体表达,可临床推广。Abstract: Objective To explore the effect of dexmedetomidine combined with sufentanil on the pain caused by burn and the expression of P2X7 receptor in monocyte. Methods A total of 152 patients in the Burns of Taizhou Hospital of Wenzhou Medical University between December 2015 and December 2017 were selected and divided into control group (76 cases) and observation group (76 cases) according to the admission order and odd-even method. The control group received sufentanil analgesia, while the observation group received dexmedetomidine combined with sufentanil analgesia. The arterial blood pressure and other basic vital signs, pain score, oxidative stress index and P2X7 receptor expression in peripheral blood mononuclear cells were compared between the two groups before and after the treatment. Results SBP, DBP and HR in the two groups were significantly decreased before and after intervention (P<0.05), but only SBP and DBP had significant differences between the two groups (P<0.05), while the difference between HR and SpO2 was not statistically significant (P>0.05). The pain scores of all periods decreased significantly (all P<0.05), and the scores in the observation group was significantly lower than those in the control group (all P<0.05). The pain scores showed a decreasing trend in each time period within the observation group and the difference was obvious (all P<0.05). The sedation scores at 20 min and 60 min after the administration were obviously different (all P<0.05), but there was no significant difference in sedation scores between the two groups at 5 min after dosing (P>0.05). In the observation group, the serum stress responses were significantly decreased (all P<0.05), but the control group was not (all P>0.05). Flow cytometry test showed that the expression of P2X7 receptor in CD14 cells was significantly decreased in the observation group (P<0.05), not in the control group (P>0.05). Conclusion Dexmedetomidine combined with sufentanil have an obvious analgesic effect on burn patients, without inhibition of the heart function, and it can significantly reduce the stress response and the expression of P2X7 receptor, should be clinically popularized.
