Clinical study of Tianma Gouteng Decoction combined with betahistine in patients with vertigo

摘要: 目的 探讨天麻钩藤饮联合倍他司汀对眩晕症患者的疗效及对经颅多普勒(TCD)参数、血液流变学和内皮素-1(ET-1)的影响。 方法 选择浙江中医药大学附属第二医院2018年3月—2019年3月期间收治的眩晕症患者142例,采用随机数字表法随机分为对照组(71例)与治疗组(71例)。对照组患者采用倍他司汀治疗,治疗组在对照组基础上联合天麻钩藤饮治疗。2组疗程均为14 d。比较2组治疗疗效,治疗前后眩晕障碍调查量表(DHI)评分和眩晕症状评分、TCD参数、血液流变学和ET-1水平变化及不良反应。 结果 治疗组总有效率(92.96%)高于对照组(73.24%),差异有统计学意义(P<0.05)。2组治疗后DHI评分和眩晕症状评分较治疗前降低(均P<0.05);治疗组治疗后DHI评分和眩晕症状评分低于对照组(均P<0.05)。2组治疗后基底动脉、左椎动脉和右椎动脉脑血流速度较治疗前升高(均P<0.05);治疗组治疗后基底动脉、左椎动脉和右椎动脉脑血流速度高于对照组(均P<0.05)。2组治疗后血浆黏度、红细胞压积和红细胞聚集指数较治疗前降低(均P<0.05);治疗组治疗后血浆黏度、红细胞压积和红细胞聚集指数低于对照组(均P<0.05)。2组治疗后血清ET-1水平较治疗前降低(均P<0.05);治疗组治疗后血清ET-1水平低于对照组(P<0.05)。 结论 天麻钩藤饮联合倍他司汀对眩晕症患者疗效良好,可改善患者脑血流动力学和血液流变学,且可降低ET-1水平。Abstract: Objective To explore the curative effect of Tianma Gouteng Decoction combined with betahistine on vertigo patients and its influence on transcranial doppler(TCD) parameters, Hemorheology and endothelin-1(ET-1). Methods The 142 patients with vertigo admitted to our hospital from March 2018 to March 2019 were randomly divided into control group(71 cases) and treatment group(71 cases) by random number table method. The patients in the control group were treated with betahistine and the patients in the treatment group were treated with Tianma Gouteng Decoction on the basis of the control group. Both groups were treated for 14 days. The therapeutic effects, DHI score, vertigo symptom score, TCD parameters, Hemorheology and ET-1 level changes, and adverse reactions were compared between the two groups before and after treatment. Results The total effective rate of the treatment group(92.96%) was higher than that of the control group(73.24%)(P<0.05). After treatment, DHI score and vertigo symptom score in the two groups were lower than those before treatment(P<0.05); DHI score and vertigo symptom score in the treatment group were lower than those in the control group(all P<0.05); the cerebral blood flow velocity of basilar artery, left vertebral artery and right vertebral artery in the two groups were higher than that before treatment(all P<0.05). After treatment, the cerebral blood flow velocity of basilar artery, left vertebral artery and right vertebral artery in the treatment group were higher than that in the control group(all P<0.05); the plasma viscosity, hematocrit and erythrocyte aggregation index in the two groups were lower than those before treatment(P<0.05), while the plasma viscosity, hematocrit and erythrocyte aggregation index in the treatment group were lower than those in the control group(all P<0.05). The serum ET-1 level in the two groups was lower than that before treatment(P<0.05), and the serum ET-1 level in the treatment group was lower than that in the control group(P<0.05). Conclusion Tianma Gouteng Decoction combined with betahistine has a good effect on vertigo patients. It can improve cerebral hemodynamics and hemorheology, and reduce the level of ET-1, which is worthy of clinical reference.
