Early levels and significance of serum amyloid A and D-dimer in children with mycoplasmal pneumonia

摘要: 目的 比较并分析难治性支原体肺炎(refractory mycoplasma pneumoniae pneumonia,RMPP)和普通支原体肺炎(mycoplasma pneumoniae pneumonia,MPP)患儿血清中淀粉样蛋白A(serum amyloid A,SAA)和D-二聚体早期水平,协助医生对RMPP患儿早期诊断。 方法 随机抽取在2018年1月—2019年6月期间入住安徽省儿童医院呼吸内科且病程在7 d内,初步诊断为支原体肺炎的患儿共52例,入院后立即抽取静脉血进行了SAA和D-二聚体水平的检测。对以上患儿给予阿奇霉素治疗,治疗后7 d内若患儿体温正常,咳嗽症状缓解或复查胸部影像学检查示:病灶较治疗前吸收,归纳为普通MPP组;反之则归纳至RMPP组。将2组入院时检测的SAA和D-二聚体水平结果分别与正常标准值进行比较,并比较2组检测结果。 结果 普通MPP组患儿早期SAA水平为(11.56±5.96)mg/L,D-二聚体水平为(0.61±0.32)μg/mL,分别与正常标准值比较,差异均无统计学意义(均P>0.05);RMPP组患儿的早期SAA水平为(42.85±35.25)mg/L,D-二聚体水平为(2.82±2.06)μg/mL,均高于正常标准值,差异有统计学意义(均P<0.05);RMPP组患儿早期SAA及D-二聚体水平与普通型MPP组比较,明显升高(P<0.05)。 结论 支原体肺炎患儿血清中早期SAA和D-二聚体水平升高提示病情易进展至难治性支原体肺炎。Abstract: Objective To compare and analyze serum amyloid A and D-dimer in children with refractory mycoplasma pneumoniae pneumonia and mycoplasma pneumoniae pneumonia and to help clinicians to diagnose RMPP children early. Methods A total of 52 children with a diagnosis of mycoplasma pneumonia who were admitted to the Department of Respiratory Medicine of Anhui Provincial Children's Hospital from January 18 to June 19 were randomly selected. Venous blood was collected immediately after admission for SAA and D-dimer of polymer levels. The above-mentioned children were treated with Azithromycin. If their body temperature was normal, cough symptoms were relieved or chest imaging examination was rechecked, the lesions were classified into ordinary MPP group; On the contrary, they were classified into RMPP group. The results of SAA and D-dimer were compared with the normal standard values, and the results of the two groups were compared. Results The early SAA level of children in the general MPP group was(11.56±5.96) mg/L, and the D-dimer level was(0.61±0.32) μg/mL. There was no statistically significant difference from the normal standard values(all P>0.05). The early SAA level of children in the RMPP group was(42.85±35.25) mg/L, and the D-dimer level was(2.82±2.06) μg/mL. There were statistically significant differences from the normal standard values(all P<0.05). The serum levels of SAA and D-dimer in the RMPP group were higher than the normal MPP group. The difference was statistically significant(P<0.05). Conclusion The increase of serum SAA and D-dimer levels in children with mycoplasma pneumonia indicates that the disease is easy to progress to RMPP.
