Clinical study on treatment of acute gouty arthritis with cross needle knife combined with modified suction cupping

摘要: 目的探讨十字针刀结合改良抽吸罐治疗急性痛风性关节炎的价值。方法选取2016年1月—2018年12月在舟山市妇幼保健院治疗的急性痛风性关节炎患者60例，按照随机数字表法分为治疗组和对照组，每组30例。治疗组采用十字针刀结合改良抽吸罐治疗，2次为1个疗程；对照组采用双氯芬酸钠缓释胶囊，50 mg/次，3次/d；2组均连续治疗14 d。比较2组临床疗效，采用视觉模拟评分法对比2组患者治疗前后疼痛情况，使用酶联免疫吸附法检测患者炎症因子水平。结果治疗后治疗组临床总有效率为96.67%，高于对照组的70.00%，差异有统计学意义(χ²=7.680，P=0.006)；治疗后治疗组VAS评分为(1.03±0.43)分，低于对照组的(3.38±1.01)分(t=11.726，P < 0.001)；治疗组CRP、IL-1β、IL-6分别为(6.23±1.36) mg/L、(3.37±0.58) pg/L、(6.56±2.08) pg/L，低于对照组的(11.41±3.21) mg/L、(5.57±1.01) pg/L、(9.87±1.74) pg/L(t=8.138，P < 0.001；t=10.346，P < 0.001；t=6.685，P < 0.001)。结论十字针刀结合改良抽吸罐治疗急性痛风性关节炎临床疗效显著，能够明显缓解患者疼痛。

Abstract: Objective To explore the value of cross needle knife combined with improved suction cup in the treatment of acute gouty arthritis. Methods Sixty patients with acute gouty arthritis who were treated in Zhoushan Maternity and Child Health Hospital from January 2016 to December 2018 were randomly divided into the treatment and control groups, with 30 cases in each group. The treatment group was treated with cross-needle knife combined with modified suction cupping, with 2 times as a course of treatment. The control group was treated with diclofenac sodium sustained-release capsules, 50 mg/time, 3 times/day. Both groups were treated continuously for 14 days. The clinical effects of the two groups were compared. The visual analogue scoring method was used to compare the pain of the two groups before and after treatment, and the enzyme-linked immunosorbent method was used to measure the level of inflammatory factors in the patients. Results After treatment, the total clinical effective rate of the treatment group (96.67%) was higher than that of the control group (70.00%), and the difference was statistically significant (χ²=7.680, P=0.006). The VAS score of the treatment group after treatment was (1.03±0.43) points, lower than that of the control group [(3.38±1.01) points, t=11.726, P < 0.001]; CRP, IL-1β, IL-6 of the treatment groups after treatment were (6.23±1.36) mg/L, (3.37±0.58) pg/L, (6.56± 2.08) pg/L, lower than the control group [(11.41±3.21) mg/L, (5.57±1.01) pg/L, (9.87±1.74) pg/L; t=8.138, P < 0.001; t=10.346, P < 0.001; t=6.685, P < 0.001)]. Conclusion The cross-needle knife combined with the modified suction cupping exerts a significant clinical effect in the treatment of acute gouty arthritis, which can significantly relieve the pain of patients.
