5E rehabilitation program based on MDT model effect of intervention on elderly maintenance hemodialysis patients

  • 摘要:   目的  探讨多学科诊疗团队模式的5E康复方案对老年维持性血液透析患者的影响,为5E康复方案的制定提供临床依据。  方法  选择2021年1月—2022年12月在淮北矿工总医院血透室收治的老年维持性血液透析患者82例为研究对象,采用随机数字法分为对照组40例和观察组42例。对照组实施血液透析常规护理干预,观察组在常规护理的基础上实施基于多学科诊疗团队模式的5E康复方案。比较2组患者干预前后自我管理能力、治疗依从性和营养状态的差异,比较观察组干预前后透析效果指标变化。  结果  组间比较显示,干预后3个月和6个月观察组患者自我管理行为4个维度评分及总分均高于对照组(P < 0.05),治疗依从性总体改善程度优于对照组(P < 0.05),血红蛋白、血清白蛋白、前白蛋白和BMI均高于对照组(P < 0.05)。组内比较显示,干预后3个月和6个月患者的血钾、血磷、肌酐和尿素氮[(21.25±4.13)mmol/L、(18.86±3.92)mmol/L vs. (23.43±4.35)mmol/L]均较干预前下降(P < 0.05)。  结论  基于多学科诊疗团队模式的5E康复方案在老年维持性血液透析患者临床应用中,能提升患者的自我管理能力和治疗依从性,对改善营养状态和提高透析效果有一定的作用。
