Correlation analysis of the inflammatory burden index with the severity of Guillain-Barré syndrome

  • 摘要:   目的  探讨新型炎症标志物炎症负荷指数(IBI)与吉兰-巴雷综合征(GBS)患者病情严重程度的关系及其预测价值。  方法  回顾性选取徐州医科大学附属医院2014年1月—2022年9月首诊为GBS的患者160例,根据Hughes功能分级量表(HFGS)评分将其分为轻型组(52例)和重型组(108例),收集2组患者一般资料、IBI及传统炎症指标,并进行统计学分析。采用ROC曲线评估IBI对重型GBS的预测价值;采用多因素logistic回归分析探讨重型GBS的影响因素。  结果  重型组患者入院时的IBI为13.15(4.06,56.78),明显高于轻型组[3.32(1.93,8.87)],差异有统计学意义(P<0.001);ROC分析结果显示,IBI预测重型组的ROC曲线下面积最大,为0.737(95% CI:0.660~0.815);2组中性粒细胞与淋巴细胞比值(NLR)、血小板与淋巴细胞比值(PLR)、单核细胞与淋巴细胞比值(MLR)、IBI、全身免疫炎症指数(SII)、全身炎症反应指数(SIRI)比较差异均有统计学意义(P<0.05);多因素logistic分析结果显示,IBI≥11.00(OR=3.803,95% CI:1.631~8.868)和MLR≥0.42(OR=5.697,95% CI:1.109~29.272)是发生重型GBS的独立危险因素。  结论  IBI作为一种新兴的炎症指标,其水平升高与GBS患者病情严重程度有关,且与传统炎症指标相比,IBI对于重型GBS患者具有更高的预测价值。
