艾司氯胺酮神经周围注射对乳腺癌改良根治术患者的术后镇痛效果分析

Analgesic effect of paraneural injection of Esketamine on patients undergoing modified radical mastectomy for breast cancer

  • 摘要:   目的  探讨艾司氯胺酮神经周围注射对乳腺癌改良根治术患者的术后镇痛效果,为临床制定术后镇痛方案提供参考。  方法  将2022年1月—2024年7月金华市中心医院收治的115例乳腺癌改良根治术患者依据随机数表法分成A组(57例)与B组(58例),A组不进行神经周围注射,B组给予神经周围注射艾司氯胺酮+罗哌卡因。观察2组患者的术后恢复质量[15项恢复质量量表(QoR-15) 评分]、炎症因子水平[白细胞介素(IL-6)、肿瘤坏死因子-α(TNF-α)]、术后镇痛效果(VAS评分)、睡眠质量[匹兹堡睡眠质量指数(PSQI)]、不良反应(恶心、呕吐、头晕、谵妄)发生情况。  结果  B组术后72 h的QoR-15评分[(107.42±7.46)分]高于A组[(102.17±6.95)分,P<0.05]。2组术后IL-6、TNF-α均呈下降趋势,且B组下降幅度更大(P<0.05)。2组术后静态、动态的VAS评分均呈升高趋势,但B组升高幅度更小(P<0.05)。B组术后72 h的PSQI评分[(7.15±1.24)分]低于A组[(8.23±1.41)分,P<0.05]。B组不良反应发生率为8.62%(5/58),A组为12.28%(7/57),2组比较差异无统计学意义(χ2=0.408,P=0.523)。  结论  艾司氯胺酮神经周围注射能够提高乳腺癌改良根治术患者的术后恢复质量,降低炎症反应,减轻术后疼痛感受,改善睡眠质量。
