Effects of diabetes self-management education on improving self-care abilities

摘要: 目的 探讨在糖尿病患者中开展自我管理教育对提升糖尿病患者自身护理能力的效果。 方法 选取2014年5月—2015年1月首都医科大学附属北京天坛医院内分泌科门诊诊断并收治的160例糖尿病患者作为本次实验的研究对象,通过医院医护人员在所选取的糖尿病患者中举办糖尿病知识讲座、让患者进行话题交流、设立糖尿病咨询体系、播放与糖尿病相关的影像传媒资料等一系列自我管理教育活动,对所有糖尿病患者及其家属进行糖尿病的性质、原因、危害以及如何防范等各方面进行指导教育,达到培养糖尿病患者的自我管理技能、强化糖尿病患者的自我管理意识的目的。采用糖尿病自身护理行为量表比较所有被选取的糖尿病患者在开展自我管理教育前后的自身护理能力的提升情况。 结果 所有患者在开展自我管理教育后的饮食行为、运动行为、血糖监测行为、足部护理、吸烟等项目的评分均较开展前明显升高,差异有统计学意义(t=16.235、30.142、24.363、39.125、11.217,均P<0.001),而患者的用药行为这一项目的评分在开展前后无明显改变,差异无统计学意义(t=1.128,P=0.124)。 结论 自我管理教育通过医护人员的巡访和指导以及患者和家属的密切配合,调整患者的饮食起居,从而改善糖尿病的症状以及预防并发症的发生,提高临床治疗效果。医护人员在糖尿病患者及其家属中开展自我管理教育能有效提升患者的自身护理能力和家属的陪护能力,有效地改善了患者日后的生活质量。Abstract: Objective To explore the effects of diabetes self-management education(DSME) among diabetes patients on improving their self-care ability. Methods Total 160 patients were recruited in our hospital from May,2014 to January,2015.The diabetes self-management education of the nature,cause,harm and prevention of diabetes was performed among the patients and their families through lecture,communication,consultation and related image data,to develop their self-management skills and strengthen their awareness of self-management.Summary of diabetes self-care activities(SDSCA) scale was used to compare the changes in self-care abilities before and after the self-management education by medical workers. Results The score ofeating behavior,exercise behavior,blood sugar monitoring,foot care and smoking behavior after the self-management education were higher than those before the education,the difference was statistically significant(t=16.235,30.142,24.363,39.125,11.217,P<0.001),but the score of drug taking behavior was not obviously changed before and after the self-management education,the difference was not statistically significant(t=1.128,P=0.124). Conclusion The diabetes self-management education can improve the symptoms of diabetes,prevent the occurrence of complications,and improve the clinical treatment effects.It also can effectively improve the self-care ability of patients and the accompanying ability of their family carers,and improve the quality of life.
