Changes and clinical significance of HMGB1 and neutrophil elastase in elderly patients with COPD

摘要: 目的 了解老年慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者急性加重期、稳定期的高迁移率族蛋白-1(high mobility group box 1,HMGB1)、中性粒细胞弹性蛋白酶(neutrophil elastase,NE)的动态变化和临床意义。 方法 采用酶联免疫吸附法检测56例COPD急性加重期患者、稳定期患者及35例健康老人血清HMGB1、NE水平。 结果 ①3组血清HMGB1水平分别为(12.86±3.81) ng/ml、(6.37±1.86) ng/ml、(4.13±0.87) ng/ml,NE水平分别为(5.67±0.87) nmol/L、(2.71±0.72) nmol/L、(0.86±0.12) nmol/L。②急性加重期组患者血清HMGB1、NE水平、FEV1、FEV1占预计值百分比显著高于稳定期组和健康对照组,两两比较差异有统计学意义(均P<0.05)。③稳定期组血清HMGB1、NE水平、FEV1、FEV1占预计值百分比高于健康对照组,差异有统计学意义(P<0.05)。④HMGB1水平与NE水平呈显著正相关(r=0.918,P<0.05),HMGB1水平与肺功能指标呈显著负相关(r=-0.923、-0.929,均P<0.05)。NE水平与肺功能指标呈显著负相关(r=-0.927、-0.912,均P<0.05)。 结论 炎症机制在COPD发病中有重要作用。测定HMGB1和NE水平为老年COPD患者病情变化的诊治提供较好的实验室依据,且对COPD致病机制研究提供依据。

Abstract: Objective To investigate the dynamic changes and clinical significance of high mobility group box 1(HMGB1) and neutrophil elastase (NE) in the elderly patients with both acute stage COPD and stable COPD. Methods Fifty-six elderly patients with both acute stage COPD and stable COPD were investigated as the study group,while thirty-five elderly healthy people were investigated as the control group.The expression of serum HMGB1 and NE were measured by ELISA in the three groups. Results ①The level of HMGB1 in three groups were (12.86±3.81) ng/ml,(6.37±1.86) ng/ml,(4.13±0.87) ng/ml,and the level of NE in three groups were (5.67±0.87) nmol/L,(2.71±0.72) nmol/L,(0.86±0.12) nmol/L.②The level of HMGB1 and NE in the elderly patients with acute stage COPD was significantly higher than the elderly patients with stable COPD and the control group,which had statistical significance (P<0.05).③The level of HMGB1 and NE in the elderly patients with stable COPD were higher than that in the control group,with statistical significance (P<0.05).④The level of HMGB1 and NE was in significantly positive correlation (r=0.918,P<0.05).The level of HMGB1 was inversely related to pulmonary function index (r=-0.923,-0.929,P<0.05).The level of NE was inversely related to pulmonary function index (r=-0.927,-0.912,P<0.05). Conclusion Inflammatory mechanisms played an important role in the pathogenesis of the COPD.Detecting the change of HMGB1 and NE in elderly COPD patients played very important basis on the clinical diagnose and the pathogenic mechanism.-
