Effect of butylphthalide sequential therapy on neurological function,inflammatory factors,Hcy and NSE levels in patients with acute progressive cerebral infarction

摘要: 目的 探讨丁苯酞序贯治疗对急性进展性脑梗死患者神经功能、炎症因子及同型半胱氨酸(Hcy)、神经元特异性烯醇化酶(NSE)的影响。 方法 选取2014年1月-2016年6月宁波市鄞州第二医院收治的急性进展性脑梗死患者86例,采用随机数字表法分为观察组和对照组。对照组43例,采用抗血小板等常规治疗;观察组43例,在对照组常规治疗的基础上,加用丁苯酞治疗。2组治疗时间均为4周。观察比较2组神经功能、炎症因子及Hcy、NSE的差异。 结果 治疗2、4周后2组神经功能缺损评分(NIHSS)均较治疗前显著下降(均P<0.05),治疗4周后与治疗2周后相比,2组NIHSS评分均显著降低(均P<0.05),且治疗2、4周后观察组NIHSS评分降低更显著(均P<0.05);治疗4周后2组炎症因子白介素6(IL-6)、肿瘤坏死因子α(TNF-α)水平均较治疗前显著下降(均P<0.05),且观察组IL-6、TNF-α水平降低更显著(均P<0.05);治疗4周后2组同型半胱氨酸(Hcy)、神经元特异性烯醇化酶(NSE)水平均较治疗前显著下降(均P<0.05),且观察组Hcy、NSE水平降低更显著(均P<0.05)。 结论 丁苯酞序贯治疗可明显改善急性进展性脑梗死患者神经功能,减轻炎症反应,降低Hcy、NSE水平。Abstract: Objective To investigate the effect of butylphthalide sequential therapy on neurological function,inflammatory factors,and Hcy and NSE levels in patients with acute progressive cerebral infarction. Methods A total of 86 patients with acute progressive cerebral infarction in our hospital from January,2014 to June,2016 were enrolled and randomly divided into control group (43 cases) and observation group (43 cases).The patients of the control group received anti-platelet therapy,etc.,while the patients of the observation group received conventional therapy and additional butylphthalide sequential therapy.The treatment time was 4 weeks in both groups.The effect of butylphthalide on neurological function,inflammatory factors,and Hcy and NSE levels were compared before and after treatment between the two groups. Results The NIHSS scores were significantly reduced at 2 and 4 weeks after the treatment in both groups (P<0.05).The NIHSS scores of both groups were significantly reduced at 4 weeks after the treatment than that at 2 weeks after the treatment (P<0.05).The NIHSS scores of the observation group were significantly reduced than that in control group after the treatment (P<0.05);Compared with the levels before the treatment,the serum levels of IL-6 and TNF-α decreased significantly at 4 weeks after the treatment in both groups (P<0.05),and the serum levels of IL-6 and TNF-α of the observation group decreased more significantly than those in the control group (P<0.05).Compared with that before treatment,the serum levels of Hcy and NSE decreased significantly at 4 weeks after the treatment in both groups (P<0.05),and the serum levels of Hcy and NSE of the observation group decreased more significantly than those in the control group (P<0.05). Conclusion The butylphthalide sequential therapy can significantly improve neurological function,restrain inflammation,and reduce the levels of Hcy and NSE in patients with acute progressive cerebral infarction.
