Effect of high-dose gamma globulin combined aspirin on T lymphocyte subsets,vascular endothelial growth factor,and inflammatory factors of children with Kawasaki disease

摘要: 目的 探讨大剂量丙种球蛋白(IVIG)加阿司匹林(ASP)治疗川崎病的临床疗效,及对川崎病患儿T淋巴细胞亚群、血管内皮因子、炎症因子水平的影响。 方法 选择行大剂量IVIG加ASP治疗的36例川崎病患儿病例为患儿组,同期36例健康儿童为健康组。检测患儿T淋巴细胞亚群(CD4+、CD4+/CD8+、CD8+)、血管内皮相关指标[血管内皮生长因子(VEGF)、内皮抑素(ES)、内皮一氧化氮合酶(eNOS)]、血清炎性因子[肿瘤坏死因子α(TNF-α)、超敏C-反应蛋白(hs-CRP)、白介素-10(IL-10)、血清降钙素原(PCT)],并与健康儿童进行比较;统计观察治疗效果。 结果 治疗后,患儿CD4+、CD4+/CD8+均较治疗前明显下降(P<0.05),CD8+水平较治疗前明显升高(P<0.05),治疗后患儿VEGF、ES水平较治疗前明显下降(P<0.05),eNOS水平较治疗前明显升高(P<0.05)。治疗后,患儿TNF-α、hs-CRP、IL-10、PCT水平均较治疗前明显下降(P<0.05),治疗总有效率为88.9%。 结论 大剂量丙种球蛋白加阿司匹林有助于改善患儿的免疫功能、血管内皮因子、炎症反应,阻止病情发展,治疗效果满意。Abstract: Objective To investigate the effect of high-dose gamma globulin (IVIG) combined aspirin (ASP) on T lymphocyte subsets,vascular endothelial growth factor,and inflammatory factors of children with Kawasaki disease. Methods A total of 36 cases of children with Kawasaki disease patients treated with high-dose IVIG combined ASP were assigned into observation group,and 36 cases of healthy children in the same period into control group.The T lymphocyte subsets[CD4+、CD4+/CD8+、CD8+)],vascular endothelial related indicators[vascular endothelial growth factor (VEGF),endostatin (ES),endothelial nitric oxide synthase (eNOS)],and serum inflammatory cytokines[tumor necrosis factor-α(TNF-α),high-sensitivity C-reactive protein (hs-CRP),interleukin-10(IL-10),serum procalcitonin (PCT)]of both groups were tested and compared.The clinical effect was evaluated and compared. Results After the treatment,CD4+ and CD4+/CD8+ of the observation group were significantly decreased as compared with those before the treatment (P<0.05),CD8+ level was significantly increased (P<0.05).After the treatment,VEGF and ES levels of the observation group were significantly decreased as compared to those before the treatment (P<0.05),eNOS level was significantly increased (P<0.05).After the treatment,the levels of TNF-α,hs-CRP,IL-10 and PCT were decreased significantly (P<0.05).After the treatment,the total effective rate was 88.9%. Conclusion High-dose gamma globulin combined aspirin can improve immune function,vascular endothelial growth factor and inflammation factor of children with Kawasaki disease,and slow the progression of disease with a satisfaction effect.
