Influence of acetylcysteine combined glucocorticoids on immune function and inflammation of patients with severe asthma

摘要: 目的 探讨乙酰半胱氨酸联合糖皮质激素治疗重度哮喘的临床效果及其对患者免疫功能及炎性因子的影响。 方法 以随机数字法将2015年8月—2016年4月杭州市红十字会医院收治的70例重度哮喘患者随机分为2组,每组35例。对照组给予糖皮质激素,观察组在此基础上加用乙酰半胱氨酸。治疗前后,检测患者免疫功能、炎性因子、呼吸道功能,并统计2组的治疗效果。 结果 治疗后,2组CD3、CD4、CD4/CD8、IgA、IgG和IgM水平均较前明显升高(P<0.05),CD8水平较前明显下降(P<0.05),且观察组CD3、CD4、CD4/CD8、IgA、IgG和IgM水平显著高于对照组(P<0.05),CD8水平显著低于对照组(P<0.05)。治疗后,2组TNF-α、hs-CRP和IL-8水平均较前明显下降(P<0.05),且观察组TNF-α、hs-CRP和IL-8水平显著低于对照组(P<0.05)。治疗后,2组PaO2、FVC、FEV1、FEV1%、FEV1/FVC水平均明显升高(P<0.05),Pa CO2明显下降(P<0.05),且观察组Pa O2、FVC、FEV1、FEV1%、FEV1/FVC水平明显高于对照组(P<0.05),PaCO2明显低于对照组(P<0.05)。治疗后,观察组治疗总有效率为91.3%,显著高于对照组的74.3%(P<0.05)。 结论 乙酰半胱氨酸联合糖皮质激素能显著改善重度哮喘患者的免疫功能、炎症、呼吸道功能,治疗效果满意。Abstract: Objective To investigate the clinical effects of acetylcysteine combined glucocorticoids on severe asthma and its influence on the immune function and inflammation, respiratory of patients. Methods Total 70 patients with severe asthma were randomly divided into two groups, with 35 cases in each group. The patients in the control group were given corticosteroids, while the patients in the observation group were given acetylcysteine combined glucocorticoids. Before and after the treatment, the immune function, inflammatory cytokines, respiratory function of the patients in the two groups were detected and the clinical efficacy was assessed. Results After the treatment, CD3, CD4, CD4/CD8, IgA, IgG and IgM levels of both groups were significantly increased (P < 0. 05), CD8 levels of both groups were significantly decreased (P <0. 05); CD3, CD4, CD4/CD8, IgA, IgG and IgM levels of the observation group were significantly higher than those of the control group (P < 0. 05), CD8 level of the observation group was significantly lower than that of the control group (P <0. 05). The total effective rate in the observation group was 91. 3%, which was significantly higher than that in the control group (74. 3%, P < 0. 05). Conclusion Acetylcysteine combined glucocorticoids could significantly improve immune function, inflammation, respiratory function of severe asthma patients, with satisfaction treatment.
