Analysis of homocysteine and blood lipid levels in patients with chronic obstructive pulmonary disease

摘要: 目的 研究同型半胱氨酸(Hcy)在不同分期慢性阻塞性肺疾病(COPD)患者中的变化及其与血脂的相关性,探讨Hcy在COPD中的可能致病作用。 方法 以2015年1月—2016年6月于北京积水潭医院呼吸内科住院的COPD急性加重期患者77例、同期门诊就诊的稳定期COPD患者61例和门诊健康查体者353例为研究对象,抽取静脉血检测Hcy、血脂、血清肌酐(sCr)、尿酸(UA)、超敏C反应蛋白(hs-CRP),比较各组以上指标的变化,并分析Hcy和血脂的相关性。 结果 COPD急性加重期、稳定期和健康对照组3组之间年龄、性别构成差异均无统计学意义。COPD患者无论急性加重期还是稳定期Hcy、hs-CRP水平均高于健康对照组(均P<0.05),并且COPD急性加重期Hcy、hs-CRP水平高于稳定期(均P<0.05)。COPD急性加重期TG、TC较稳定期、健康对照组均下降(均P<0.05),LDL-C也明显下降(P<0.001);COPD稳定期LDL-C与健康对照组相比下降(P<0.05),但TG、TC水平差异无统计学意义(均P>0.05)。COPD急性加重期sCr较健康对照组升高(P<0.01)。相关性分析显示COPD无论急性加重期还是稳定期Hcy水平与血脂、sCr、UA、hs-CRP均无明显相关。 结论 COPD急性加重期Hcy水平明显升高。COPD患者易合并心血管疾病、代谢综合征可能是Hcy升高的结果。Abstract: Objective To study the changes and relationship of homocysteine(Hcy) and blood lipid levels in patients with different stages of chronic obstructive pulmonary disease(COPD) and explore the role of homocysteine in COPD patients. Methods From January,2015 to June,2016,77 cases of inpatients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD),61 cases of outpatients for stable COPD,and 353 healthy volunteers were enrolled in this study.The serum Hcy,hs-CRP,blood lipid,serum creatinine(sCr),and uric acid(UA) were detected and analyzed.The correlations with the study parameters were investigated. Results There was no statistical difference in three groups with age and gender.The Hcy and hs-CRP levels were significantly higher in AECOPD and stable COPD group compared to healthy controls(Hcy:P<0.001 and P=0.034;hs-CRP:P<0.001 and P=0.028).Additionally,Hcy and hs-CRP levels in AECOPD group were higher than that of stable COPD group(P=0.040 and P=0.009).The levels of TG and TC in AECOPD group were lower compared to stable COPD group and the healthy controls(TG:P=0.017 and P=0.001;TC:P=0.002 and P<0.001).LDL-C levels in AECOPD were significantly lower compared to stable COPD group and healthy controls(both P<0.001).Furthermore,the LDL-C level in stable COPD group was significantly lower compared to healthy controls(P=0.041),but there was no statistical difference of TG,TC level(P=0.844 and P=0.093).sCr level in AECOPD group was higher than that of healthy controls(P=0.004).There was no relationship between Hcy and blood lipid,sCr,UA,hs-CRP levels both in AECOPD and stable COPD group. Conclusion The Hcy levels in patients with AECOPD increased.Some comorbidities such as cardiovascular diseases and metabolic syndrome,maybe the result of increased serum Hcy levels in COPD patients.
