Role of CT-guided three-dimensional puncture positioning device in the management of sympathetic modulation

摘要: 目的 探讨CT定位穿刺三维引导器在交感神经调制手术中的应用价值。 方法 选择2015年1月—2016年1月在浙江省嘉兴市第一医院接受胸交感神经调制术或腰交感神经调制术的患者60例。采用随机数字表法,随机选取患者的一侧采用CT定位穿刺三维引导器引导穿刺法(引导法),另一侧采用CT定位常规穿刺法(常规法),穿刺点为T3~4或L2~3椎间隙,穿刺针分别抵达T4肋骨小头上方或L3椎体前外侧与腰大肌前方。记录2种方法患者术前(T0)、术后10 min (T1)、术后24 h (T2)的掌温、PI值及穿刺时间、穿刺次数、穿刺时针干与设定穿刺角度的偏差、穿刺相关并发症的发生情况。 结果 患者在T1时刻的手指或脚趾末梢灌注指数平均上升了(7.73±1.23)倍,掌温平均上升了(4.97±0.94)℃。电话随访结果显示,所有患者疗效均能持续3个月以上。与常规法比较,引导法穿刺时间显著缩短[(14.85±3.27) min vs.(35.35±3.10) min,P<0.001];穿刺次数显著减少[(3.25±0.91)次vs.(8.65±1.79)次,P<0.001];穿刺时针干与设定穿刺角度的偏差降低[(3.70±1.22)° vs.(12.20±2.58)°,P<0.001]。所有患者成功完成穿刺,无血胸、气胸等并发症发生。 结论 CT定位穿刺三维引导器可缩短穿刺时间,提高穿刺成功几率,值得临床推广。Abstract: Objective To investigate the values of CT-guided three-dimensional puncture positioning device in the management of sympathetic modulation. Methods From January,2015 to January,2016,60 patients proposed for thoracic or lumber sympathetic modulation in our hospital were enrolled into study.According to the principle of completely random digital table,one side was punctured under the guidance of CT-guided three-dimensional puncture positioning device(guided technique),while the other side was punctured under the CT-guided conventional puncture technique(conventional technique).Thoracic intervertebral puncture was performed between T3 and T4 till the needle tip reached the spot around T4 and above capitis;Lumber intervertebral puncture was performed between L2 and L3 till the needle tip reached the lateral side of L3 and anterior side of the psoas muscle.The temperature of hands or feet、the patient's finger or toe tip perfusion index were recorded before the operation(T0),10 minutes and 24 h(T1,T2)after operation.Also,the time consumed during puncture,CT scan adjusting time,the deviation of the needle from the targets and the incidence of puncture complications were recorded. Results In two Groups,perfusion index and the temperature of hands or feet raised (7.73±1.23) times and (4.97±0.94)℃ in average,respectively.All of the patients were cured within 3 months.When compared with the conventional technique,guided technique got a significantly shorter time consumed during puncture[(14.85±3.27)min vs. (35.35±3.10)min,P<0.001],the number of attempts and the deviation of the needle from the target were significantly reduced[3.25±0.91 vs. 8.65±1.79,P<0.001],[(3.70±1.22)° vs. (12.20±2.58)°,P<0.001].The procedure was successfully performed on both side in all patients without serious complications such as hemothorax and pneumothorax. Conclusion CT-guided three-dimensional puncture positioning device in the management of sympathetic modulation can shorten the puncture time,improve the success rate of puncture,it is worthy of clinical promotion.
