Effect of calcitonin as an adjuvant therapy on pain,bone mineral density and bone metabolism indexes in senile osteoporosis patients with low back pain

摘要: 目的 老年性骨质疏松患者多伴发腰背痛,患者往往因为害怕骨折以及腰背疼痛而拒绝锻炼,从而加重骨质疏松,本研究旨在探讨降钙素辅助治疗对老年性骨质疏松所致腰背痛患者疼痛、骨密度(BMD)及骨代谢指标的影响。 方法 选取嵊州市人民医院2016年2月—2017年1月于嵊州市人民医院进行治疗的老年性骨质疏松所致腰背痛患者80例,随机对照表法分为观察组和对照组,各40例。对照组采用常规治疗,观察组在对照组的基础上加用降钙素进行辅助治疗,2组患者均治疗6周。比较2组患者治疗效果及治疗前后腰背视觉模拟评分(VAS)、骨密度及骨代谢指标变化情况。 结果 观察组患者治疗总有效率为92.50%,对照组患者治疗总有效率为75.00%,观察组患者治疗总有效率明显高于对照组(P<0.05);2组患者治疗后腰背VAS评分均较治疗前显著降低(P<0.05),且观察组患者腰背VAS评分降低更显著(P<0.05);2组患者治疗后腰椎(L)2-4 BMD、股骨颈BMD均较治疗前显著升高(P<0.05),且观察组患者L2-4 BMD、股骨颈BMD升高更显著(P<0.05);2组患者治疗后骨代谢指标1,25-二羟维生素D3[(1,25-(OH)2D3)]较治疗前显著升高(P<0.05),甲状旁腺素(PTH)显著下降(P<0.05),且观察组患者1,25-(OH)2D3升高及PTH下降更显著(P<0.05)。 结论 降钙素辅助治疗可明显增强治疗效果,减轻老年性骨质疏松患者腰背痛,改善其骨密度及骨代谢情况。Abstract: Objective The low back pain commonly occurs in patients with senile osteoporosis.The patients often refuse to exercise due to fear of fracture and back pain,thus exacerbating osteoporosis.This study aims to investigate the effect of calcitonin as an adjuvant therapy on pain,bone mineral density(BMD) and bone metabolism indexes in senile osteoporosis patients with low back pain. Methods A total of 80 senile osteoporosis patients with low back pain were divided into observation group(40 cases) and control group(40 cases) from February,2016 to January,2017.The patients of the control group received conventional therapy,while the patients of the observation group additionally received calcitonin therapy based on control group.The treatment time was 6 weeks.The clinical effect and changes of pain,BMD and bone metabolism indexes were compared between the two groups before and after treatment. Results The total effective rate of the observation group was 92.50%,and of the control group was 75.00%,total effective rate of the observation group was significantly higher than that of the control group(P<0.05);The VAS score of low back pain decreased significantly after treatment in both groups(P<0.05),and VAS score of the observation group decreased more significantly than that of the control group(P<0.05);The BMD of L2-4 and femoral neck increased significantly after the treatment in both groups(P<0.05),and the BMD of L2-4 and femoral neck of the observation group increased more significantly than that of control group(P<0.05);The level of 1,25-(OH)2D3 of bone metabolism indexes increased significantly after the treatment in both groups(P<0.05),the level of PTH decreased significantly after the treatment in both groups (P<0.05),and the difference in the level of 1,25-(OH)2D3 and PTH between the two groups was significant(P<0.05). Conclusion Calcitonin as an adjuvant therapy can improve the clinical curative effect,reduce low back pain,and regulate BMD and bone metabolism in senile osteoporosis patients with low back pain.
