Investigation and analysis of the difference between the expectation value and the actual value of the family doctor contract service of the elderly over 60 years old

  • Objective To analysis the existing problems of the contracted service of family doctors and put forward countermeasures and suggestions through the evaluation of the expected value and the actual value of the family doctor contract service of the elderly over 60 years old. Methods Using the convenient sampling method, 420 contracted residents over 60 years old who admitted in the community health service center of Datuan, Pudong New Area from February to April 2018 were selected as the survey objects. A self-designed questionnaire was used to investigate the expected and actual values of the contracted residents' family doctor contract service. The data were statistical analyzed. Results Total of 420 questionnaires were issued, 400 questionnaires were recovered, with an effective recovery rate of 95.24%. In 400 cases, the awareness rate of family doctors signed by family doctors was 91.75% and satisfaction was 90.25%. For the 12 services of basic medical treatment, public health and agreed health management provided by family doctors, the expected score was more than 8 in 10 items(83.33%), and the actual perception was more than 8 in 6 items(50.00%). The top 5 service items with the largest difference were long prescription(9.27±1.35 vs. 6.81±1.79), on-site service(9.40±1.42 vs. 7.50±2.52), family sickbed(9.24±1.30 vs. 7.57±2.51), extended prescription(9.16±1.13 vs. 7.60±1.33), and health education(9.34±1.45 vs. 8.02±2.03). Conclusion The actual value of 12 basic medical services, public health services and agreed health management services provided by the respondents to the family doctors was higher, but there was a certain gap between them and the expected value.
