Objective To explore the effect of family doctor individual health management strategy in the management of patients with type 2 diabetes mellitus.
Methods Total 149 patients with type 2 diabetes were selected and randomly divided into the traditional group of 73 cases and the individual management group in 76 cases in Shanghai.The traditional group was given the responsibility system of family doctors,while the individual management group was implemented the individual strategy management.The patient satisfaction of both groups before and after the management and the results of satisfaction survey were compared.
Results After the management of 6 months,the bad habits rate of patients in the traditional group was not improved(
P>0.05),while in the individual management group was significantly improved(lower than that before the management and the rate of traditional group(
P<0.05).Blood glucose,blood lipid and body mass index (BMI) situation: each index of the traditional group were slightly lower than before the management,the difference was not statistically significant(
P>0.05);while each index in the individual management group were significantly better than those of the traditional group and the rate before the management,the differences were statistically significant(
P<0.05).The satisfaction with family doctor team services was 88.16% in the individual management group and 72.60% in the traditional group,the difference was statistically significant(
P<0.05);the patients satisfaction with the disease control was 77.63% in the individual management group and 47.95% in the traditional group,the difference was statistically significant(
P<0.01).
Conclusion Based on the individual situation of patients,the family doctor individual strategy can more effectively promote the health behavior of patients with diabetes,improve the patient’s condition,promote the harmonious relationship between doctors and patients.It is worth to promote the application of family doctor in chronic disease management.
