Objective The works in Pharmacy Intravenous Admixture Services(PIVAS) were with the characteristics of multiple processes,high risk and high intensity. 6S model management was introduced into the PIVAS safety management process,in order to reduce the internal errors of PIVAS,improve work efficiency,strengthen the safety management,and ensure the safety of clinical patients intravenous drug usage.
Methods The study analyzed the internal error of PIVAS and the causes before the implementation of 6S management during the period of January to June and July to December in2012 retrospectively. Firstly,6S management implementation team was set up in PIVAS,and the training for 6S management knowledge was performed in all staffs to strength the significance and importance of 6S management,summarized the exists potential safety hazard by the method of brainstorming,and perfected the corresponding workflow. 6S management(including seiri,seiton,seiso,seiketsu,shitsuke and safety) was implemented daily from July,2012 to December,2012 strictly. The effect such as work efficiency and work quality after the implementation of 6S mode in the PIVAS were evaluated.
Results The entire quality of PIVAS service was improved distinctly after the implementation of 6S mode,especially in decreasing the rate of five common internal error of PIVAS(wrong labels,wrong order of drugs,errors in label printing,wrong dose or medicine,and omissions of labels),
P < 0. 05,and the efficiency and department satisfaction were improved.
Conclusion 6S mode can improve the working conditions and staff professionalism,reduce the occurrence of internal errors,and ensure the safety and efficiency of configuration of intravenous admixture.
