Objective To explore the value of color Doppler spectrum and quantitative contrast enhanced ultrasound on evaluation of renal microcirculation before and after treatment of chronic glomerulonephritis.
Methods Fifty cases of chronic glomerulonephritis in our hospital between January,2014 and December,2015 were enrolled and divided into good prognosis group and poor prognosis group according to the therapeutic effect.The levels of blood urea nitrogen,serum creatinine and 24-hour urine protein were tested before and after the treatment.The renal arterial resistance index (RRI) and maximal blood flow velocity were measured by color Doppler spectrum.The mean transit time,area under the curve,peak intensity and time to peak were measured by contrast-enhanced ultrasonography.
Results There was no significant difference of blood urea nitrogen,serum creatinine,24 h urinary protein,The long axis of the kidney and the thickness of the kidney after treatment and those before treatment (
P>0.05).There was no significant difference in the resistance index and the maximal velocity of segmental artery after treatment and those before treatment (
P>0.05).The area under the curve after treatment was higher than that before treatment (
P<0.05).Before treatment,there were no significant differences in the mean transit time,peak time,area under the curve and peak intensity between the two groups (
P>0.05).After treatment,the mean transit time,peak time,area under the curve and peak intensity of the good prognosis group were higher than that of the poor prognosis group (
P<0.05).There was a negative correlation between the difference of urinary protein before and after treatment and the area under the curve before and after treatment (
r=-0.901,
P<0.001).
Conclusion There are no significant changes in biochemical parameters and color Doppler spectrum parameters before and after treatment in patients with chronic glomerulonephritis.Contrast-enhanced ultrasound can be sensitive to the changes of renal microcirculation in patients with chronic glomerulonephritis before and after treatment.
