Research management strategies for multimorbidity in elderly patients

  • 摘要: 当前全球人口老龄化严重,我国也进入了老龄化的加速阶段。老龄化伴随而来的共病问题给经济社会的发展带来了严峻挑战,共病问题的出现严重影响了老年患者的生活质量,同时也造成了多重用药、治疗负担的加重,随着患慢病数目的增多,更有部分患者伴随着心理疾病的困扰,同时, 老年共病还可以引起住院次数增加, 这直接导致了医疗成本和资源利用率大幅增加, 所以共病问题逐渐成了一个社会健康问题。慢性病的共病管理也成了“健康中国2030”关注的重点,目前慢病管理的指南多集中在处理单一疾病,而患有共病的老年人常容易被忽视,许多临床实践表明,针对处理单病的指南并不适合共病问题的解决,为了更好地管理共病患者,国内外学者进行了大量研究,2012年美国老年医学会发布了《老年共病患者的诊疗指导原则》,提出了以患者为中心、针对老年共病患者的诊治策略,该原则为我国共病管理提供了参考思路和理论框架;国内学者在共病研究中也积累了相关经验,强调通过实行多学科整合团队,发展全科医生制度,采取以患者为中心的管理模式,自我管理模式,重视老年综合评估,重视多重用药等更好地管理共病。本文对共病的概念、流行病学、共病的危害及共病管理等展开论述,以期为国内老年共病患者的健康管理起到积极的推动作用。
