Immunomodulatory mechanisms of acupuncture in the treatment of immune thrombocytopenia

  • 摘要: 免疫性血小板减少症(immune thrombocytopenia, ITP)是一种由免疫介导的获得性自身免疫性疾病,该病以血小板计数显著降低及出血风险增高为主要临床特征。当前糖皮质激素、静脉丙种球蛋白等ITP一线治疗方案虽能短期控制病情,但依旧存在不良反应多、易复发及部分患者疗效不佳等诸多临床难题。针刺作为中国传统医学的重要非药物疗法之一,具有整体调节与多靶点干预的特点,可通过神经-内分泌-免疫网络调节机体免疫稳态,在ITP的辅助治疗中具有重要研究价值。针刺在先天免疫层面,可调控抗原呈递细胞的功能状态,抑制其过度活化与促炎反应,从而减轻对自身抗原的异常呈递;在适应性免疫层面,针刺能双向调节淋巴细胞稳态,既抑制B细胞的抗体生成功能,减少致病性自身抗体的产生,又纠正T细胞亚群的失衡,在降低效应T细胞过度活化的同时,增强调节性T细胞的免疫抑制功能。本文通过梳理及分析近十年针刺调节免疫以及ITP免疫发病机制的相关文献,系统总结了针刺治疗ITP的潜在分子与细胞机制,将零散证据进行系统整合,以期阐明针刺通过多细胞、多通路协同恢复免疫稳态的核心路径,在为临床针刺治疗ITP提供理论依据的同时,进一步明确针刺干预ITP的可能机制,最终为临床优化治疗方案提供科学依据。
