Application of multi-round multi-module grading method in laparoscopic visual reality skill training

摘要: 目的 在应用腹腔镜模拟手术技能培训系统对不同层次学员进行腹腔镜微创手术技能培训的过程中,采用多轮多模块分级训练法针对不同学员进行分层训练,本研究即探讨多轮多模块分级训练法的实际训练效果,并进行经验总结。 方法 本研究所采用腹腔镜手术操作模拟培训设备包括贝德思达医学训练模拟器和Simbionix手术模拟训练系统2个部分。培训分为多轮进行,每轮又分为基础模块(第1模块)、中级模块(第2模块)、高级模块(第3模块)。受训学员从第1轮第1模块开始培训。经过一定学时(6学时)的培训后接受考核,通过考核可晋级进入更高一级模块培训,未能进入下一模块的学员则继续第2轮相同模块培训,通过考核后方可进入第2轮更高一级模块培训。依此类推,如果第2轮仍没有成功晋级,则继续进行第3轮相同模块的培训。每轮培训3个模块全部通过为培训合格。培训合格学员最后再次经过专家点评后结业。 结果 全部参训人数55人,培训分为3轮进行,受训学员从第1轮第1模块开始培训。经过每人6学时的培训后接受考核,28人通过考核晋级进入第1轮第2模块培训,未晋级者则继续第2轮第1模块培训。16人通过考核后进入第1轮第3模块培训,没有晋级学员继续第2轮第2模块培训直至晋级。第1轮培训16人(学时不限)3个模块全部通过,培训合格。培训合格学员最后再次经过专家点评后结业。第2、3轮培训依此类推,通过3轮多模块分级训练法55位学员全部培训合格。不同临床经验的学员经过培训后均能够熟练掌握在模拟操作设备上的基本手术操作技能,其中70%以上的学员可以逐步向临床过渡。 结论 在应用腹腔镜模拟手术培训中采用多轮多模块分级训练法对学员进行培训,可以使不同临床经验学员的腹腔镜基本手术操作技能都有大幅提高,为学员真正走向临床打下坚实基础。Abstract: Objective To explore the effect of multi-round multi-module grading training method in laparoscopic visual reality surgical skill training in participants with different clinical experiences. Methods The training was divided into different round and module.There were basic module (the first module),middle level module(the second module) and advanced module(the third module) in each round.The training started from the first module of the first round training.After a certain hours(6 hours) of training,the assessments should be taken,the doctors who pass the exam came into a higher level module training in the same round,the doctors who failed to enter the next module still learning in the corresponding module of the second round until they pass the exam and then came into the next module in the same round.So,if it is still no successful promotion in the second round,then the doctors continued to practice in the corresponding module of the next round.The training completed after passing three modules for certain round and receiving the final review by the surgical experts. Results Total 55 doctors participated in 3 rounds training.The training started from the first module of the first round.Every doctor accepted the assessment after a 6 hours training.28 trainees passed the examination and then entered into the second module of the first round training.After another 6 hours training,16 trainees passed the examination and entered into the third module of the first round training.16 trainees passed the three modules of the first round training.The qualified students will joint in the final expert review at last.After the same training way at the second and the third round of training,all the 55 doctors passed the training.After the multi-round multi-module grading skill training in laparoscopic visual reality operation exercises,the sense of space position and operation handle feel,skillful using of instruments and the ability of both hand-eye and hand-hand had improved significantly of the doctors with different clinical experiences.More than 70% of the passing students can transit to clinical practice gradually. Conclusion In the laparoscopic visual reality skill training,application of the method of multi-round multi-module grading training,the laparoscopic operation skills of the students with different clinical experience improved obviously,and laid a solid foundation for the students for the approaching clinical practice.
