Objective To study the short-term curative efficacy of laparoscopic radical gastrectomy on advanced gastric cancer patients and its effect on intraperitoneal micrometastasis.
Methods A total of 113 patients with advanced gastric cancer in our hospital from March,2012 to March,2016 were divided into laparoscopic group and laparotomy group according to the treatment methods.The patients in the laparoscopic group received laparoscopic radical gastrectomy,and patients in laparotomy group received traditional open stomach cancer radical surgery.The perioperative indicators,treatment situation,dopamine decarboxylase(DDC) and Carcinoembryonic antigen(CEA) of intraperitoneal washings,complications of the two groups were compared.
Results The incision length,intraoperative blood loss,analgesic drugs number,anal exhaust time,firstly getting out bed time,feeding diet time,hospitalization time of laparoscopic group were significantly lower than those of laparotomy group,the difference was statistically significant(
P<0.05).The swept lymph nodes number,distance from near cut to tumor,distance from far-cut to tumor of laparoscopic group had no significantly difference to laparotomy group(
P>0.05).After the treatment,the DDC and CEA of the two groups were significantly increased,the difference was statistically significant(
P<0.05),but DDC and CEA of intraperitoneal washings of the laparoscopic group were significantly lower than those of the laparotomy group,the difference was statistically significant(
P<0.05).The complication of laparoscopic group was significantly lower than that of laparotomy group,the difference was statistically significant(
P<0.05).
Conclusion Laparoscopic radical gastrectomy for advanced gastric cancer is effective,with the characteristics of small surgical trauma,low incidence of peritoneal micrometastasis,and high security.
