The relationship between bone mineral density and bone metabolism in postmenopausal patients with type 2 diabetic nephropathy

  • Objective To investigate the relationship between bone mineral density(BMD) and bone metabolism in postmenopausal patients with type 2 diabetic nephropathy and to analyze influencing factors of BMD. Methods According to urinary albumin excretion condition,a total of 94 patients with type 2 diabetic were divided into pure diabetes group(53 cases) and diabetic nephropathy group(41 cases) from Oct.2014 to Sep.2016 admitted in our hospital.To survey clinical data,BMD and bone biochemical markers were determined and BGP,PTH,PINP,β-CTX were compared in 2 groups.At the same time,to investigate the relationship between BMD and bone metabolism. Results Course,parathormone(PTH) and Type I collagen peptide carboxyl end beta special sequence(β-CTX) of diabetic nephropathy group were significantly higher than those in pure diabetes group(P<0.001).BMD,bone gla protein(BGP) and Type I collagen amino terminal before the peptide(PINP) of diabetic nephropathy group were significantly lower than those in pure diabetes group(P<0.01);to control age,course,menopause age and body mass index,partial correlation analysis showed that BMD were negatively correlated with PTH,PINP and β-CTX in post menopausal patients with type 2 diabetic nephropathy(P<0.05);Multi-factor linear regression analysis showed that,body mass index,menopausal period,PINP,β-CTX are the main influencing factors of BMD in postmenopausal patients with type 2 diabetic nephropathy(P<0.05). Conclusion Compared with postmenopausal pure diabetes patients,postmenopausal patients with type 2 diabetic nephropathy patients had longer duration of existence,low BMD,bone metabolic abnormalities,PTH,PINP,and beta CTX,which can more sensitively reflect the BMD,and the BMD of patients with type 2 diabetic nephropathy can be evaluated via body mass index,duration of menopause,PINP,and beta CTX.
