Objective To explore the effect of transition care for discharged elderly patientson their quality of life,satisfaction and readmission rates.
Methods Total 92 old patientsin two tertiary hospitals inWuhan were chosen and divided into two groups according to the random number table before the discharge.There were 46 people in control group and 46 people in experimental group.Both groups received the same general nursing,while the experimental group received additionalindividual transition care according to the situation of the rehabilitation through telephone follow-up on 1st,2nd,4th and 6th after the discharge.All participants were investigated by the questionnaire of quality of life scale(QOL-BREF) and the nurse satisfaction on the discharge day and the second and sixth mouth after the discharge.All the data were analyzed by using
t test and χ
2 test.
Results There were no significant difference in the scores of the elderly’s quality of life(

P=0.234),the satisfaction of the nursing(

P=0.424) and the rate of readmission(0,0) on the discharge day and so did the second mouth(

P=0.079),between the two groups,but the scores on the sixth mouth after the discharge in the experimental group were significantly higher than those in the control group(

Conclusion Transitionalcare can effectively improve the quality of life in elderly after discharge,decrease the readmission rates,reduce the use of medical resources,andincrease patients satisfaction with the nursing care.
